I'm working a denial for place of service not allowed for 36821 at POS 11. My question is where do you find (check) or where does it state if a procedure may or may not be perform in a paticular POS.

If I understand your question correctly, you are asking where do you find the POS services in the CPT manual. You will find the POS Codes in front of your CPT manual. It has three columns--Place of Service Code(s), Place of Service Name, and Place of Service Description. The codes are listed in numerical order (01, 02, 03, etc.).

No that's not my question. Im aware of the POS codes. the question is there are certain procedures that cms states shouldn't be done in an office setting. Im trying to see where or how do I verify which can or can't. and if they are actually done in an office setting should the provider contact the health plan for approval prior to doing the procedure?

Maybe if you go the CMS website, you may find the answer and I am sorry for misunderstanding your question. Some of those documents on CMS are pretty big. I'll look on the CMS website for you and see if I can find some links to the POS question.
